PARENTAL CONSENT
FOR MINORS
TO ATTEND THE
GREAT LAKES COMIC CONVENTION
The form below is required for anyone under 18 years of age attending the Great Lakes Comic Convention who is not accompanied by an adult.
Minors ages 13-17 may attend the event without an accompanying adult - HOWEVER, they must have the Parental Consent Form completed and signed to attend the event.
All children aged 12 and under must be accompanied by an adult aged 18 or older at all times while on the premises of The Great Lakes Comic Convention.
The completed form must be presented at the Box Office in order to be admitted to the event.
